Millennium High School is happy to offer the opportunity for student-athletes to have their sports physicals completed here on campus prior to the start of the 2022-2023 school year.  This gives student-athletes a chance to get ahead on their paperwork, and serves as a convenience for those that may have difficulties making it to an appointment elsewhere in the midst of work, school, and sports schedules.

Sports physicals on campus are $20, conducted by Dr. Andy Su and team.  Payment and permission slips are due by Wednesday May 25th to Miss Balsamo’s box in the front office.  This deadline is important to help Dr. Su ensure he is properly staffed based on the number attending.  The permission form can be found under "Links/Forms".
